yoga therapy terms and conditions

Release of Liability

You as a client must stipulate that you are under the care of a physician or other health care provider for your medical conditions. It is further agreed that Doctor Atoosa Kourosh shall not be liable for injuries or damages to your person or property arising out of, or connected with, the use of her yoga therapy services. You must agree to consult with a physician before starting any program or intervention discussed and also agree to inform Doctor Atoosa Kourosh of any relevant health conditions at the outset of interaction and as new developments arise.

You must also stipulate that Doctor Atoosa Kourosh is your yoga therapist and not necessarily your physician, and that yoga therapy is not a substitute for appropriate medical care from your own doctor. While Doctor Atoosa Kourosh may advise you on relevant yoga-related health practices as well as access your health information based only on your consent, she is not responsible for the state of your health, with the exception of your also being a patient in her medical practice.

Payment Policies

Full payment is expected prior to the time of your appointment, per the terms of the service or program package you have selected.

All payments for services rendered are final; refunds are not available.

We accept payment through HSA and FSA cards, credit cards, Zelle, Stripe, Venmo, CashApp, PayPal, cash, money order, or cashier’s check.

We do not bill insurance, but can provide upon request detailed insurance claims forms for services rendered that you can submit to your HSA, FSA, or insurance company for credit toward your deductible, maximum out-of-pocket expenses, or reimbursement. Please contact your HSA, FSA, or insurance provider with any questions about submitting your claims paperwork.

If your account is over 30-days past due, you will receive a letter stating that you have 10 days to pay your account in full. Partial payments will not be accepted unless otherwise negotiated. Please be aware that if a balance remains unpaid, we may refer your account to a collection agency.

cancellation Policies

Please notify Doctor Atoosa Kourosh 24-hours in advance of a set meeting if you need to cancel. Same-day cancellations will still be charged for the scheduled session.
